This page outlines the work we are doing at the long-term conditions population board to support people who are living with long term conditions

Long-term conditions (LTCs) are illnesses that cannot be cured. They are health conditions that can impact a person’s life and may require ongoing care and support.

Long-term conditions are a major cause of poor quality of life in England. People in poorer areas are more likely to have long-term health conditions, and these conditions tend to be more severe than those experienced by people in less deprived areas. Deprivation also increases the likelihood of having more than one long-term condition at the same time. On average people in the most deprived areas of the population develop multiple long-term conditions 10 years earlier than those in the least deprived areas.

People from diverse ethnic groups are more likely than white British people to report limiting long-term illness and poor health, with those identifying as white Gypsy and Irish Traveller reporting the poorest health.

The Long-Term Conditions Population Board brings together partners from across Leeds to tackle long-term illness. This will help us tailor better care and support for individuals and their carers, design more joined-up and sustainable services and make better use of public resources.

The LTC population includes people aged 18 and over, not at end-of-life or frail, and with at least one diagnosed long-term condition (for example, heart disease, diabetes, arthritis, and asthma).

Outcomes for the long-term conditions population board

The ambition of our long-term conditions work in Leeds is that we will improve support for people managing LTCs and their carers. The following ambitions outline what we want to achieve as a board:

  1. I’m as healthy and as well as I can be with my long-term condition(s)
  2. People with a LTC return to and maintain their normal activities and lifestyle in ways that matter to them
  3. People with a LTC take an active role in managing their condition

These are our identified outcomes. By setting these clear goals, that are focused on how services impact the people they serve, the board is able to better track whether we’re really doing the right thing for the people using these services.

Making changes to services for long-term conditions

The ICB in Leeds is committed to achieving the best value for taxpayers’ money, and remaining accountable to the public, communities, and patients it serves. In line with this commitment, we are publishing our Quality and Equality Impact Assessments (QEIAs). These assessments describe how proposed changes to services or policies might affect people in Leeds. They also support the ICB in Leeds to identify opportunities to reduce the impact of any changes to services.

We are actively working to publish our Quality and Equality Impact Assessments on this site where possible, and to ensure that these documents meet accessibility and data protection standards. Public involvement workshop
